Title V - Online as of 10/01/23
TITLE V related Permit Applications and Projects
Effective October 1, 2023, the Health Department will transition and process all Title V* related projects via an electronic online system called Open Gov.
Open Gov is a web-based system that allows the applicant to apply and track the Title V related projects and licensing process from start to finish.
First-time users will need to create an account, then can apply for a permit or license electronically.
Payment Methods:
- Credit Card (customer pays processing fee)
Pay with Visa, Mastercard, Discover and American Express - Cash
- Check made payable to: Town of Shrewsbury
- E-Check (customer pays processing fee)
Pay via ACH direct-debit from most major banks, with instant verification via login to online banking.

The Title V Repository is a centralized location broken down by street address to access pump reports, septic plans, Title V inspection reports, septic repairs, abandonments, etc.

Abandonment application. Copy of pump slip (not required if system is dry). $75.00 - check payable to: Town of Shrewsbury The Title V Agent requires a minimum of 48 hours prior to requesting witnessing of crush and back-fill. |
Perc / Soil Test* | Perc / Soil Test application*. $400.00 per lot - check payable to: Town of Shrewsbury Effective 12/01/21 - Perc / Soil Test application fees are non-refundable and non-transferrable. A $400.00 per lot cancellation fee will apply if canceled and not rescheduled less than 72 hours prior to the scheduled inspection date/time. |
Repair or Upgrade | DEP Form 1A / 2A / 3. Sketch. Certificate of Compliance. $225.00 - check payable to: Town of Shrewsbury (Fee includes $125 for the inspection and $100 for COC) |
Plan Review Disposal System Construction Permit (DSCP) |
DEP Form 1A / 2A / 3. 4 copies of stamped septic plans. Soil log (or the multi-part state form). Perc test results. $675.00 - check payable to: Town of Shrewsbury. (Fee includes plan review ($250, inspections ($325), as-built and COC ($100)) DEP Form 9A / 9B if Local Upgrade Approval requested. Certificate of Compliance. |
Septic Installer Permit | New applicant needs to complete a Septic Installer application. $325.00 - check payable to: Town of Shrewsbury. Proof of insurance. Copy of three (3) septic installer permits from other towns. |
Approval of septic plan extensions |
Septic plans expire three (3) years from the date of approval. A letter requesting a one (1) year extension must be submitted prior to the plan's expiration date. |
Do you have an on-site disposal system and are considering purchasing an inground or above-ground swimming pool?
Below, please find information regarding Title V setbacks:
In-ground pools must be located at least 20 feet from leach fields and at least 10 feet from septic tanks. Above-ground pools don't have specific required setbacks from leach fields and septic tanks, but it is recommended that they not be placed on top of the leach field.
