All applicants must complete the following forms:
- Application Requirements as noted in the specific application (see above)
- Shrewsbury Application for License (see above)
- Worker's Compensation Insurance Affidavit (see above)
- Original Hard Copy of All Submitted Licensing Documents, One (1) Additional Copy, and One (1) Digital Copy (emailed or on disk/ USB)
Please contact the Office of the Board of Selectmen for all additional requirements for the license in question, and complete the proper application.
Once a license has been reviewed and approved by the Board of Selectmen, the License and regulations for the particular license must be posted on the premises in a conspicuous place.
