Vital Records

Birth Certificates

Birth records are available for individuals who were born in Shrewsbury or whose parents resided in Shrewsbury at the time of the birth. 

Death Certificates

Death records are available for persons who died in Shrewsbury or whose residence at the time of death was Shrewsbury.

Marriage Certificates

Marriage records up to 1979 are available for persons who resided in Shrewsbury at the time of marriage or who were married in the town of Shrewsbury. Records from 1980 to present are available for couples who obtained a marriage license and filed their marriage intention in Shrewsbury.

How to request a certified copy:

If you are unsure if a record exists, please confirm with the Town Clerks Office prior to ordering by calling 508-841-8507.

  1. Visit www.shrewsburyma.gov/payclerkonline
  2. Select the transaction
  3. Enter the required information and submit the payment (fees and postage applied)
  1. Fill out the information in the Town of Shrewsbury Vitals Request Form  
  2. Mail the completed form, with proper payment (check or money order), and self addressed and pre-stamped envelope to the Town Clerks Office

  1. It is recommended to call the Town Clerk's Office ahead so we may prepare the record for you and ensure its availability. 
  2. Visit the Town Clerk's Office to pick up your record and submit payment of $10 per copy (cash, check, money order and credit card accepted - fee applies to credit card payments)
Your request will be mailed to the address provided.
Your request will be mailed to you in the self-addressed and pre-stamped envelope as provided
Your request will be made available at the time of request at the counter. 


The fee for a certified copy  of a vital record is $10.  (cash, check, money order and credit card accepted - fee applies to credit card payments)
